ORDER FORMSBelow are links to various Service Order Forms for your information and
use. We ask that you please anticipate your additional show requirements and complete the
enclosed forms at your earliest convenience. This will enable us to have the requested
items delivered to your booth prior to your arrival.
We suggest completing and returning the
Credit Card Authorization Form to protect your exhibiting representative from being responsible for any balance due on
site. All orders returned by fax must be accompanied by a completed Credit Card Authorization
Form in order to be processed.
NOTE: All order forms should
be returned to Blue Chip Expo. Questions concerning electrical, internet and AV service should be directed to Keri Rounsavall,

Exhibitors may begin set up of individual booth displays on Wednesday, March 21st at 1:00PM until 4:00PM.
Exhibitors must begin dismantling
individual booth displays on Friday, March 23rd at 11:00AM. The exhibit hall must be completely cleared by 4:00PM

event. We ask that you please anticipate your additional show requirements and please read the information carefully. This
will enable us to have requested items delivered to your booth prior to your arrival. If you have questions or need
